Paperwork Deadline Looms

The following paperwork is due by July 17 and can be mailed to Judy Frewin or turned in at team camp the week of July 10.

Forms Needed:
Physical Form
Citizenship Card
Athletic Trainer Form (If the new physical form is used, this not needed)
Player Info Sheet
Copy of insurance card (front and back) on one piece of paper
